In this packet you will find information that will help you as you plan for your student teacher. I’ve included specific information about: 

  • general policies
  • your role as a cooperating teacher

    Student Teacher Evaluations:
        Midterm Evaluation Form  

Midterm Evaluation Instructions:
Click on the above link to fill out the midterm evaluation form.  Once the form has been completed, please print, sign and submit to the University Supervisor on the third observation visit of your student teacher. 

This report should describe the student teaching competencies that have been observed by the cooperating teacher during the first half of the full-time directed teaching experience. The midterm report is used to help identify any strengths or weaknesses in the performance areas stated on the enclosed evaluation form. In essence, it should indicate to the student teacher “how he or she is doing” at this point in time. The midterm report is to be signed and mutually shared with the student teacher.

Final Evaluation Instructions:
Click on the above link to fill out the final evaluation form.  Once the form has been completed and reviewed with the student teacher, please print, sign and submit to the Teacher Education University Supervisor on the final observation visit.

The final student teaching evaluation should describe the directed teaching competencies that have been observed by the cooperating teacher and the university supervisor. The final evaluations become a part of the student’s permanent file and may be used to respond to requests by potential employers. Since many people may view the student’s file, the contents of the final evaluation are important.

Please keep in mind that our students are at the entry level of the teaching profession and are not finished products. Your summary comments should briefly describe the student teacher’s skills and personal qualities.

The completion of directed teaching does not complete the process - it only helps to ensure a successful beginning. We appreciate your endeavors, encouragement, patience, and wisdom.
