Student Injury and Sickness Insurance - Plan Information
Eligibility for Student Insurance
- Traditional Undergraduate students with 6 hours or more are and Traditional Graduate students with 5 or more hours are automatically covered.
- Coverage begins August 15, each year (and covers anywhere in the world), and termination depends on your semester attendance at Cornerstone (see insurance brochure for further information)
- Travel immunizations are covered at cost, maximum of $200 ($10 co-pay per visit), as long as they are done on campus, not at a health department. A student insurance claim form must be completed. The form is available in Health Services as well as on this site under "Forms"
Limitations and Reductions
- Co-pays: Most Health Services co-pays are $10 per visit. Details are listed in the brochure under the "forms" tab.
- Outpatient Referral - Physician visits, including medical centers, when referred by Cornerstone University Health Services medical staff is covered up to $250 maximum with a $10 co-pay per visit.
- On-campus Weight Watchers is covered up to $200 per year with co-pay dependent on the length of the program.
- Injuries and illnesses may be cared for on campus in Health Services. Charges for these are covered up to $300 per year.
- There is also a benefit in our Counseling Services Department with up to 10 sessions per year. For details see the Counseling Department section.
Pre-existing health problems and routine health issues are not covered. There is no vision, dental, or prescription benefit.
