SDS Procedure
If you have a disability that necessitates academic accommodations or specific supports on campus, you must complete the following steps:
- Contact the SDS at the Cornerstone University Learning Center, located in Miller Hall.
- Complete an Application for Services, including signing a release that allows related professionals access to records. All records are kept confidential.
- Provide thorough documentation of your disability. Documentation must be : provided by a psychologist, physician, or other specialist in your area of disability; provide a specific diagnosis; and include specific recommendations for academic adjustments and accommodations.
- Documentation will be reviewed by our Accommodations Officer and the Accommodations Review Committee(ARC). All students must have signed a release to allow these reviews.
- After documentation has been reviewed and accepted, you will meet with the SDS Director to develop your Individualized Student Accommodation Plan (ISAP). The ISAP outlines your specific accommodations and is an agreement between you and Cornerstone University.
Please note: The Accommodations Review Committee (ARC) meets monthly, and to be reviewed on the ARC’s agenda for any given month, students must have all documentation submitted by the 1st day of that month.
